UPS Worldport Disability Reports

Recently there has been a rash of employees facing termination at UPS due to failure to completely follow all steps of the disability reporting procedure. The two largest problems at this time are

Employees do not have their current address on file with UPS, resulting in them not receiving crucial correspondence from UPS.

Employees are not submitting each and every doctor’s note keeping them off of work to HRSC and the company.

In order to protect yourself while you are out on disability, please follow ALL the following steps in their entirety! Failure to do so can result in your termination at UPS!

When going out on disability please follow these steps:

  1. Update your address on UPSers.com, obtain a disability form from the Teamsters Local 89 Union Hall and notify HRSC at 1-855-877-4772.
  2. Fill out the first portion of this form yourself, have your doctor fill out the second, and have management fill out the third.
  3. Return the form to the Union Hall and have the form faxed to TeamCare and HRSC.
  4. Notify TeamCare, HRSC, and your management team every time you go to the doctor. Do this by fax and/or e-mail and keep copies of fax confirmations and emails. Any phone conversations should be by cell phone and those records should be kept. Failure to keep HRSC and the company up to date with any and all doctor’s notes keeping you off of work will lead to termination for unauthorized leave!
  5. Any correspondence from UPS or HRSC should be answered immediately. Do not ignore certified mail delivery attempts from the United States Postal Service. Remember, you have 10 days to file a grievance over a termination.
